Nursing and midwifery navigation

The Nursing and Midwifery Navigation Service aims to provide those with complex health issues support in managing their health conditions. Navigation Services places you at the center of care, meaning staff can help you navigate your health in both the hospital and community. By providing clinical expertise Nursing and Midwifery Navigators help build your ability to take control of your health. By working across the hospital and community, Navigator clinicians are able to help reduce fragmentation between health care providers, external services, hospitals and various appointments.

The West Moreton Health Navigation service is delivered by a team of specialist Nurses, Nurse Practitioners and a Midwife. Navigators work closely with your team of health care providers to establish a central point of contact. This ensures all of your health care services know where you are on your health care journey.

Nursing and Midwifery Navigators work with you to establish what health care needs you have, where you would like to be with your health in the future and provide tailored education on your current medical conditions. Additionally, Nursing and Midwifery Navigators work with you to bring services together to ensure you get the right care, in the right place and the right time.

Who can refer?
  • External Service Providers (NGO etc)
  • WMH clinical staff
  • GPs.
How to refer

Internal and External referrals to the Navigation service are accepted via the following pathways:

  • Completion of the Navigation referral form
  • Phone call to Navigation service – an intake process will be undertaken.
