Perineal disease and faecal incontinence


+ Emergency

If any of the following are present or suspected, please refer the patient to the emergency department (via ambulance if necessary) or seek emergent medical advice if in a remote region.

  • Suspected strangulated/incarcerated or obstruction of any hernia
  • Acute, severe abdominal pain with or without associated sepsis
  • New onset of obstructive jaundice
  • Acute painful perianal conditions
  • Acute cholecystitis
  • Gallstones with symptoms of cholangitis
  • Acute pancreatitis
  • Bowel obstruction
  • Severe per rectum bleeding
  • Acute abscess at any site
  • Acute testicular pain

+ Other management information

  • Local application of cold packs and oral anti-inflammatory medications
  • Referral to pelvic floor physiotherapist
  • Education about pelvic floor care and specific techniques for defecation
  • Oral antibiotics such as Augmentin®, or clindamycin where penicillin allergy is a factor, should be considered.
  • Sexual counselling for the couple
  • Counselling for subsequent pregnancy management
  • Importance of follow-up six weeks and three months postpartum
  • Postpartum management: avoid constipation, use of aperients, dietary advice
  • Reassurance and provide support – psychological

    Fistula

  • Persisting fistula discharge/infections Glyceryl trinitrate 0.2% ointment
    (Rectogesic®): TDS for four-six weeks (NS)
  • Lifestyle modification (increased activity, dietary, weight, smoking, alcohol)

    Pilonidal Sinus

  • Amoxicillin and clavulanic acid is the preferred antibiotic.
  • Lifestyle modification (increased activity, dietary, weight, smoking, alcohol)
  • Advice on hygiene, sweating activity, activity associated with sitting and buttock friction

  • Previous gastroenterologist investigations and results (date, report and histology results) e.g. last 2-3 clinic letters
  • Management to date including timeline, medication and lifestyle
  • History of previous drainage operation

  • Assess sphincter function and integrity and endoanal USS results

Out of catchment

West Moreton Health is responsible for providing a public health service to people who reside within its catchment area. To appropriately manage demand for service we do not accept referrals from outside this catchment area. If your patient does live outside the West Moreton Health area and it is deemed socially or clinically necessary for their care to be received in the West Moreton Health Service, inclusion of information regarding their particular medical and/or social factors will assist with the triaging of your referral.

Named referrals

If you would like to send a named referral, please address it to the specialist on the referral template, who will allocate a suitably qualified specialist to see the patient.

From July 1 2017 Commonwealth growth funding has been capped. This changes how WMHS can fund its growth as an organisation. Named referrals from GP’s help support hospital funding through a Medicare bulk-billing arrangement. The new federal funding model incorporates specific pricing for patients which removes concerns around ‘double dipping'. This benefits hospital and patient services.
