We want everyone in West Moreton to get the health care they need.

We know that where you live, the language you speak, your health history, and your responsibilities at work and home are among many things that affect how and where you need to access care.

Through service improvement, and training and support for our staff, we are ensuring health care is inclusive and easy to use.

Why planning matters

We are developing two plans to help us prioritise work that will make care easier to access and improve health outcomes for our diverse communities.

The plans will align to our strategic plan and support the Queensland Government’s plan for our communities.

Listening to our community, partners and staff will help us set the right priorities.

Disability service plan

People with disability have a right to safe, respectful, and quality healthcare.

Yet health inequities exist when unfair systems, attitudes and environmental barriers limit full participation in health services, contributing to poorer health outcomes and reduced wellbeing.

West Moreton Health is developing its Disability Service Plan 2026–29 to ensure the people most affected by barriers to access and inclusion have a say in shaping their care.

Before we can identify the right actions, we need to understand the health and service needs of people with disability across our region. Input from people with a disability, their families, carers, and the staff who support them will guide the development of this plan.

Our engagement process is underpinned by the social model of disability and acknowledges that poor health outcomes experienced by people with disability may result from factors other than their medical condition or impairment.

Health literacy plan

Understanding and acting on health information isn't equally easy for everyone.

Our new Health Literacy Plan 2026–29 aims to make it fair and easy to access healthcare.

The plan will outline what we need to do to ensure our communities can get the best out of the available services in the West Moreton region.

It will focus on how we can reduce barriers to care by making sure health information is clear and easy to find and follow, communication is consistent, and our services are easier to use.

To develop a plan that meaningfully reduces barriers, we need to hear from the people who experience them: community, carers, staff and health partners.

Our engagement process is grounded in equity, inclusion, cultural safety, and co-design, and your feedback will directly shape the priorities and actions of the plan.
