Our staff will work with you to plan your return home after your stay in hospital. This will help to ensure that you have everything you need for a full recovery when you leave us.
We want to make your move to home or another facility as smooth as possible.
Your care team will discuss discharge plans with you.
Some questions you may like to ask include:
- What is my expected date of discharge?
- What should I do if I have any concerns about my condition?
- What should I expect my recovery to involve?
- What preparations will I need to make?
- What information will be sent to my GP to assist in managing my care?
- What follow up appointments and services will I need?
Please make arrangements for your transportation home and ensure you are comfortable with any plans for ongoing care. If you need assistance with arranging transport, please let us know.
We also recommend you confirm with staff that the contact details in your medical chart are correct, such as your contact details and those for your next of kin and your GP.
You can leave hospital as soon as the discharge process has been completed.
Because safety and patient counselling are key elements of our service, the pharmacist will visit you to talk with you about your ongoing medication needs, medication changes, compliance and supply needs. You may prefer to use your regular pharmacy, buy medicines over the counter or require a supply from the hospital before you leave.
The focus of the pharmacy service is on ensuring your medication is managed, what the medicines are used for and how long to take them.
If your medication requirements are complex often the pharmacy will coordinate care plans with community pharmacies to organise monitored dose systems.
The pharmacy currently operates Monday to Friday and generally does not fill scripts after 4.00pm.
From time-to-time, the best option may be to transfer you to another ward or facility to ensure appropriate care for your needs, or to allow you to complete your recovery closer to your home. This will be discussed with you and organised for you.

- A list of your medications
- Any medications and scripts provided to you – fill any scripts for medications as soon as possible
- Any information about post hospital care and any follow up appointments
- Medical certificate if required.
