West Moreton Health Consumer Liaison (CLO) service manages, and records, all feedback received in relation to West Moreton Health Services – compliments, suggestions and complaints. We appreciate all feedback about healthcare services provided by West Moreton Health.

Provide feedback

You can access the CLO service by:

  1. Filling in a electronic feedback form

  3. Phone: 0409 275 503 (Monday - Friday, 8:00am to 4:00pm)

Raise health service concerns

The CLO team will work with you and the treating team to resolve complaints. This will include:

  1. Sharing your specific feedback concerns with the treating team, including the outcome you are seeking
  2. Working with the treating team to review your concerns and determine the best way to respond to your feedback
  3. Helping the treating team to provide you with a response.

The CLO team also records all feedback so that West Moreton Health can identify areas for improvement.
