This service provides navigation across the lifespan to those individuals who have complex medical conditions.
The West Moreton Health Navigation service aims to provide integrated care to patients with complex health conditions or disabilities across the lifespan. Navigators work across the health care continuum to reduce fragmentation and duplication of services, improve health outcomes, increase health literacy and optimize appropriate use of health resources.
Nursing and Midwifery Navigators are specialist clinicians supporting patients to reduce the impact of poor health outcomes through connection to services for timely intervention.
Nursing and midwifery navigators are highly experienced and have an in-depth understanding of the health system, contributing to improved pathways of care and building partnerships across primary, secondary and tertiary care.
