Service information

The WMH Nursing and Midwifery Navigation Service aims to provide integrated care to patients in our region. This service provides navigation across the lifespan to those individuals who have complex health or disability conditions. This service places the patient at the centre of all service delivery whereby Navigators work across the health care continuum. By providing navigation to patients with complex care needs, navigators reduce fragmentation and duplication of services, improve health outcomes, build individual patient/consumer health literacy and optimize appropriate use of health resources.

Nursing and Midwifery Navigators are specialist clinicians who work with complex patients to reduce the impact of poor health outcomes through connection to services for timely intervention. The navigation goal is to improve the patient/consumer outcomes through enhanced health literacy, personal management and connection with appropriate health services. By providing clinical expertise across the health care continuum, Navigators also contribute to building permanent pathways and relationships across the health system (primary, secondary and tertiary services) to reduce fragmentation and duplication.
