Preventative Integrated Care Service (PICS) provides rapid access (review within 24-48 hours) to intensive specialised medical management, delivered by a team of medical, nursing, and allied health clinicians, with a specific focus on supporting people with chronic conditions who require multi-disciplinary input to avoid a potential hospital presentation or admission.

PICS is a five-day service providing care over a period of 16-days, through varied modalities including home visits, outpatient appointments, and virtual care (such as telehealth and telemonitoring), depending on individual care requirements.
